Perimenopause, HRT, and Massage

If you've found yourself waking up at 3 a.m., feeling achy for no apparent reason, or wondering why your body suddenly feels unfamiliar, you're certainly not alone.


Perimenopause is a normal stage of life, but that doesn't mean it's easy. As estrogen and progesterone fluctuate, many women experience hot flashes, night sweats, sleep disruption, mood changes, joint pain, headaches, muscle tension, fatigue, and increased stress.


For many women, hormone replacement therapy (HRT), is an effective medical treatment for appropriate candidates. Massage therapy is not a replacement for HRT but it can be an excellent companion to it. The majority of my clients are on an HRT. (Massage therapists cannot prescribe medications or diagnose anyone for anything.)


Massage Doesn't Change Your Hormones


Let's start by clearing up a common misconception. Massage therapy has not been shown to increase estrogen, progesterone, or "balance" hormones. Those claims simply aren't supported by good scientific evidence.


Instead, massage works through mechanisms we understand much better:


  • Reducing muscle tension

  • Encouraging relaxation

  • Lowering perceived stress

  • Improving local circulation

  • Helping regulate the nervous system

  • Reducing local pain sensitivity


Those effects can make a meaningful difference when your body is already working hard to adapt to hormonal changes. While hormones play a role in many of these effects, massage still isn't "balancing" anything. But it can help you temporarily feel better by giving you small boosts of serotonin and dopamine. (How can you change your hormones? Talk do your doctor about medications, your diet, and recommended exercise. I'm a massage therapist, not a doctor. This is about massage.)


Better Sleep


Loss of sleep is often one of the first things to pop up during massage consultations. Many women find that relaxing deeply before bedtime helps them fall asleep more easily and feel more rested. A lot of people listen to ASMR podcasts/tiktok/youtube videos to help them drift off. Regular massage/asmr sessions can improve your overall mental health, which can also lead to better sleep routines.


Estrogen: Muscle and Joint Pain


One of the most overlooked symptoms of perimenopause is the increase in muscle soreness and joint stiffness. Estrogen plays a role in maintaining connective tissue, muscle recovery, and inflammation.


"In these other musculoskeletal tissues, estrogen improves muscle mass and strength, and increases the collagen content of connective tissues. However, unlike bone and muscle where estrogen improves function, in tendons and ligaments estrogen decreases stiffness, and this directly affects performance and injury rates." -a study from Jan 2019 cited below


So, as hormone levels fluctuate, everyday aches may become more noticeable. Massage can't stop those hormonal changes, but it can help ease muscle tension, improve mobility, and provide temporary relief from the discomfort that often accompanies them. Many women tell me they feel more comfortable moving after a session.


When Estrogen dips, so does your serotonin and dopamine, resulting is lower moods and increased stress and brain fog.


Stress Matters


Perimenopause can feel emotionally exhausting. Between interrupted sleep, changing bodies, work, caregiving, and everyday responsibilities, many women spend months or years with their nervous systems constantly "on." Massage offers something increasingly rare: uninterrupted time for your body to shift into a calmer state. That doesn't mean massage "fixes cortisol" or resets your nervous system permanently. Showers are also temporary. But giving yourself regular opportunities to relax may help reduce stress, improve mood, and create space to recover from the constant demands of daily life.


If You're Taking Hormone Therapy


Many of my clients use hormone replacement therapy while also receiving regular massage.

The two treatments work in very different ways. Hormone therapy addresses the underlying hormonal changes responsible for symptoms like hot flashes, vaginal dryness, and bone loss in appropriate candidates. Massage focuses on how your body feels by helping ease muscle tension, improve comfort, reduce stress, and support overall well-being. HRT and massage therapy are perfect complements to each other.


The Bottom Line


Massage therapy offers something valuable for women experiencing perimenopause, not because it changes your hormones, but because it helps care for the body that's adapting to them. Sometimes that's exactly what your nervous system needs.


Massage Recommendation


A monthly massage gives you a few things to look forward to, starting with the knowledge that you're allowed to take care of your body. You get to schedule time for yourself every month, which also has the added mental health benefit of something to look forward to, you get a massage, an hour or two to yourself, and space to let your body do what it needs to. Work is stressful, life is demanding, and your body is changing on a chemical level.
